UPDATE
November 22, 2011:
The issues with the Adobe LiveCycle Policy Server
have been resolved, including the ability to access the
manual with
Adobe Reader 9 and X.
SINGLE USER LICENSEES Online and offline access to
the 2011 UB-04 Manual expired 6/30/11. Users who have not
renewed will not be able to log in. Google Checkout is
the quickest way to renew (see http://www.nubc.org/become.html).
Continuing subscribers log in using their
existing User Names and Passwords. New subscribers
or subscribers with new email addresses are sent registration invitations via
e-mail. You select a password as part of the
registration process. You will not be able to view/download these
files until you register and activate your
subscription. Adobe Acrobat/Reader 7.0 and
later is required.
Change
your Password
1. Log onto https://www.ahapolicyserver.com 2. Select "Options" in upper right menu.
3. Enter new password and confirm.
Forgot
Password
The site administrator has no access to subscribers'
passwords.
If, after you have activated, you forget your password, contact policyserver@aha.org
and request a Reset Password Email. A machine generated
password will be sent that can then be changed per the instructions
above.
